Direct Support Coordinator - South Bay (Full Time)

Responsibilities
Facilitate the client’s inclusion in positive community experiences
Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel
Maintain current knowledge of client’s needs
Maintain positive communications and relationships with his/her supervisor, clients, landlords, employers, involved members of families, Regional Center, D.O.R. and other individuals in the community
Monitor and report to case responsible person on client and employee activities
Complete CSPs, monthly/quarterly reports, evaluations, intakes and assessments under the direction of the case responsible person
Prepare and maintain current staffing schedules and coordinate new staff training schedules
Implement individual training and support programs under the direction of the case responsible person
Document required activity records, reports, incidents, and client evaluations in a timely, complete, and accurate manner as required
Address challenging behaviors proactively and respectfully, using methods outlined in plans and training
Oversee client activities and environment throughout shift to ensure the fulfillment of health and safety needs
Review activity logs of clients and report significant events or concerns to case responsible person
Participate in the evaluation of the client through completion of task analysis and other requested assessment devices
Qualification
Required
18 years of age or older
Successful completion of pre-employment drug screen and background check
Insured, registered vehicle in safe working condition, and proof of current CA driver's license
High School Diploma or Equivalent
Adjoin's client case management platform requires employees to utilize a mobile smart device (i.e. cell phone/tablet) with reliable internet connectivity and data to complete certain essential job tasks
Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test
Aptitude for confidentiality, organizational skills and good interpersonal skills
Bachelor's degree or 3 years equivalent experience and/or educational equivalent in working with individuals with disabilities
This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee
Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time
Certification in an approved First Aid and CPR class; or successful completion within 45 days of the date of hire
