Program Manager – Veterans - Grant Per Diem Program (GPD)

Harbor Care is an innovative health and human services non-profit organization that provides various services to support military veterans and homeless populations. They are seeking a Program Manager to oversee the Veterans Grants Per Diem transitional housing program, ensuring compliance, managing daily operations, and leading a team to support veterans effectively.

Responsibilities

Ensure full compliance with VA GPD regulations, internal policies, and applicable federal/state laws
Oversee daily operations of three transitional housing sites, ensuring consistent, high-quality service delivery
Monitor program outcomes and conduct regular quality assurance reviews, internal audits, and site visits
Coordinate with VA GPD liaison(s), prepare for inspections and evaluations, and manage timely, accurate reporting
Maintain thorough documentation in client records and data systems in alignment with HUD and VA requirements
Supervise, mentor, and support case managers and residential staff
Conduct regular team meetings, performance evaluations, and professional development sessions
Foster a trauma-informed, Veteran-centered culture among team members
Provide coverage and leadership support for other program managers as needed
Offer 1:1 support to Veterans when appropriate, especially in crisis or complex cases
Assist staff with client service planning, crisis de-escalation, and connection to wraparound services
Promote individualized goal setting with Veterans around housing stability, employment, recovery, and wellness
Serve as a primary liaison to VA, HUD, housing authorities, service partners, and other community stakeholders
Participate in coordinated entry meetings, community coalitions, and other relevant external forums
Represent Harbor Care and the GPD program professionally in public and partnership settings
Oversees and helps to develop the activities of all the Grant Per Diem programs within Harbor Care, in alignment with Harbor Care policies and procedures and grant requirements
Input into human resource matters, budget input and management
The Program Manager will advocate for our Veterans and their needs
The Program Manager will also work collaboratively with staff to ensure that all Harbor Care services are provided and developed with a high level of quality and efficiency
In collaboration with Case Managers, work on goals to end Veteran homelessness in New Hampshire through collaboration with all other Veterans Services Programs throughout Harbor Care
This position will directly oversee the following program: GPD Transitional Housing Program
Works under the supervision of and reports regularly to the Director of Veterans Services
Ensures the safety of the staff, residents, and the facility
Familiar with HMIS and the process of inputting data, updating data as needed
Provides coverage in the facility in the event of residential staff absences
Monitor the implementation of the residential service plans and services for program residents
Works closely with VA GPD Liaison to ensure Veterans entering the program are properly screened
Communicates regularly with GPD Case Managers to assure coordination of services and compliance
Monitors the quality of all resident files and progress, and completes related documents
Oversees all monitoring by funders for the facilities
Oversees and facilitates entrances and exits from the program
Participates in the employment process and provides staff scheduling and routing assessment of staff performance
Develops and complements routine management reports and dashboards
Aids in the completion of program administrative tasks
Performs any necessary computer related activities
Manages the facility and related finances and record
Promote a culture of high performance and continuous improvement that values learning and a commitment to quality
Provide leadership in agency wide clinical and programmatic planning with VA Liaison Support
Work collaboratively with the local COC’s to ensure GPD is supported in the community efforts to end Veteran Homelessness
Coordinate programming with partner agencies, such as other non-profits, local, state, and federal
Present to large groups of people and organizations (virtually and face-to-face)
Participate in organizational meetings and committees
Deliver presentations and trainings to a diverse group of professionals
Travel to Harbor Care facilities across New Hampshire as needed
May be assigned other duties as needed

Oversees the day-to-day efficient operations of the GPD programs under the scope of this role
Provides direction and oversight to staff by coaching and mentoring to achieve department objectives in the delivery of services to Harbor Care staff and clients
In collaboration with case managers, ensures all transitional facilities have appropriate staffing coverage
Reinforces and communicates performance expectations and monitors compliance with performance standards
Sets realistic goals for staff to maximize productivity
Recognizes the merit of continuing education and encourages attendance at appropriate educational opportunities. Creates opportunities for employee participation in staff development
Facilitates the resolution of staff grievances and complaints with support from the Compliance Department
Participates in Quality Assurance and Performance Improvement activities in Harbor Care
Ensures compliance with OSHA and other safety regulations
Adhere in compliance to all HIPAA rules and regulations
Ensures that department operations follow state and federal regulations
Assists with adverse event Root Cause Analysis as indicated
In collaboration with the CFO and program managers, develop and manage the operational and/or grant budgets for the departments
Consult with other program managers in the development of grant budgets as needed
Tracks trends and make recommendations on cost savings opportunities

Required

Bachelor's or master's degree in behavioral science, Social Work, Psychology, or related field
Minimum three (3) years of experience working in a mental health or behavioral health setting
Demonstrated supervisory experience in a social services or clinical environment

Preferred

Experience working in a residential setting or transitional housing program
Direct experience working with Veterans and understanding of military culture
Familiarity with Section 8, HUD housing, and state/local benefits systems
Strong problem-solving, organizational, and communication skills
Ability to work independently and collaboratively within a team
Skilled in crisis management and conflict resolution
Confident public speaker and effective communicator across all organizational levels
Highly organized, detail-oriented, and able to manage multiple priorities
Demonstrated commitment to diversity and inclusion in all aspects of work
Veteran status preferred
