Clinical Pharmacy Specialist - PACT
The North Florida/ South Georgia VA Healthcare System's Pharmacy Service is seeking an experienced and dynamic Clinical Pharmacy Specialist-PACT. The role involves contributing to the achievement of quality care standards and the growth of a VA academic hospital program while ensuring optimal drug therapy outcomes for patients.

Responsibilities
Provide care appropriate to the patients served by the Patient Aligned Care Team (PACT)
Communication and interpersonal relations, including the ability to appropriately and courteously relate to internal and external customers as well as the ability to persuade and influence clinical and management decisions
Responsible for analyzing real and potential drug related medical problems and implementing corrective action to ensure that patients receive optimal drug therapy outcomes
Monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Provide appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises other health care providers as appropriate
Participates in team meetings, reviews charts, orders and evaluates pertinent laboratory and other tests, assesses drug-drug and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies
Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program
Extends refills until next scheduled appointment per facility policy
Serves as a role model and preceptor to pharmacy students, interns and residents
Solves problems, coordinates and organizes responsibilities to maximize outcomes within the setting of PACT

Education- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Licensure- Full, current and unrestricted Pharmacy license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia

English Language Proficiency- Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d)
must have 1 year of experience equivalent to the next lower grade level GS-12
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Must pass a pre-employment physical examination as required by VA Handbook 5019
This position requires moderate lifting up to 44 lbs., carrying objects up to 15 lbs., frequent reaching above shoulder, use of fingers, hands and legs, use of cranes and motor vehicles, walking & standing up to 8 hours, bending and climbing and able to hear whisper
The incumbent must be physically, emotionally and mentally stable in order to efficiently perform the essential functions of this position without hazard to themselves or others
