Responsibilities

Helping patients navigate through the healthcare system and providing basic care coordination
Identifying and reducing barriers to care and addressing social determinants of health
Educating patients on non-medical factors regarding how and where to access health care services
Providing assistance in gaining access to transportation for preventive/routine appointments as well as specialty appointments
Making referrals to Health Homes and Financial Services Offices and assisting with follow up to ensure patients' needs are addressed
Following up on high-risk patients to determine whether they received the needed care from Center for International Health and other indicated provider(s) as referred to

Required

Bachelor's degree in Social Work, Public Health, Human Services, Psychology, or a related field with one year of experience in a healthcare setting required
Knowledge of community resources and an understanding of medical terminology required
Excellent written and verbal communication skills required
Documentation, organizational, planning, and implementation skills with the ability to handle multiple cases and timelines required
Computer skills and knowledge including Windows and Microsoft Word required

Preferred

Understanding of social determinants of health and the needs of diverse populations preferred
Bilingual candidates preferred
Experience working with the refugee population preferred
