Clinical Pharmacy Specialist - Hematology/Oncology
The VA Northeast Ohio Healthcare System is recruiting for a Clinical Pharmacy Specialist. This position plans and provides clinical pharmacy care in oncology/hematology areas throughout the medical center; works with other pharmacists and practitioners to provide comprehensive clinical and educational services, precepts residents and students on clinical rotation for inpatient and/or outpatient Oncology/Hematology.

Responsibilities
Plans and provides patient related clinical pharmacy and drug information services in inpatient and outpatient oncology/hematology areas and takes responsible and appropriate actions to promote optimum, most cost effective, drug therapy
Functions under a scope of practice consistent with skills, training, and education, and consistent with the pharmaceutical care needs in oncology/hematology
Provides clinical pharmacy services to staff in the outpatient oncology/hematology clinics
Assists providers with pharmacotherapy issues for patients cared for by oncology/hematology specialists and trainees
Provides pharmacotherapy related evaluations and recommendations in multidisciplinary outpatient oncology/hematology visits
Completes detailed medication histories, focusing on adherence or non-adherence with prescribed regiment, identifying potential adverse effects of medications taken, and evaluating overall medication regimen as it compares to therapeutic goals, and assuring that the medication "list" is reconciled accurately with actual medication use
Reviews patient profile for appropriateness, duplication, drug/drug and drug/food interactions, incompatibilities, or other drug related problems, and either makes appropriate changes to patient's medications or recommends change to the provider who is seeing the patient
Evaluates and responds to non-formulary consult requests within 96 hours
Develops and reviews monographs, protocols, and/or guidelines for oncology/hematology pharmacotherapy for local, network, and national level on an as needed basis
Documents and reports the occurrence of adverse drug events occurring in oncology/hematology patients
Serves as drug information resource to oncology/hematology providers
Accessible by phone and able to report to medical center after hours, every third weekend, and every third holiday when needed to finish and compound necessary orders
Evaluates physician's orders as to appropriateness and contacts the provider for recommendations to optimize therapy and follow medical center standards
Completes order entry in computer system
Prepares and dispenses chemotherapy and adjunct pre-medications in accordance with requirements for hazardous and non-hazardous medication requirements including but not limited to PhaSeal system
Prepares and dispenses infusion clinic medication orders in accordance with requirements for hazardous and non-hazardous medication requirements including but not limited to PhaSeal system
Designs, coordinates and maintains dispensing records for cancer chemotherapy and investigational drugs
In-person provider clinic with real-time team dynamics and active patient care (including fellow clinic)
Pharmacist-led oral chemo clinic (new med education, short-term, long-term follow up)
Pharmacist-led post C1 infusion therapies toxicity clinic
Projects (cost-savings initiatives, process optimization, etc)
Non-formulary medication prior authorizations
Participation in APPE student and resident precepting
Qualification
Required
U.S. Citizenship
Graduates of foreign pharmacy degree programs must provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States, or the District of Columbia
Proficient in written and spoken English
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel

Preferred
Previous dedicated Hematology/Oncology Pharmacy experience and/or training
Board certification oncology pharmacist
