Helpline Youth Counseling (HYC) is one of the largest youth services providers in Southeast Los Angeles County, dedicated to empowering youth and families. They are seeking a full-time Lead Case Manager for the Thriving Together project, responsible for providing specialized case management and coordinating mental health and substance use disorder care for young adults in under-resourced areas.

Responsibilities

Provides specialized case management to ensure their co-occurring mental, social and substance use needs are adequately identified and addressed through skill development/management and community linkages-connections

Assist with increasing local level outreach to community locations in Southeast Los Angeles County serving and supporting the young adult population, including drop-in mental health centers, temporary housing shelters, continuation schools, community colleges, adult schools, law enforcement settings, and service sites including access centers

Provides culturally and linguistically appropriate communication and information to engage clients

The LCM will coordinate mental health/SUD care services, provide mental health/SUD education, and facilitate care in a timely manner

Maintain accurate and timely documentation of all job-related activities

Coordinate with external care providers and entities providing the clients with some level of care

Will be required to participate in required training and clinical supervision

Must have excellent organizational skills and must complete all required reports and documentation in a timely manner

Participate in weekly clinical supervision or case conferences that may include other members of interdisciplinary team

Performs other duties as assigned by supervisor
