Bilingual Care Coordinator - Parenting in Recovery

Responsibilities
From a Wraparound Model frame of reference, provides care coordination to program participates in a variety of setting including office, clients’ home, school and community. The Care Coordinator develops the child and family team in consultation with the family and the FDTC-PIR team. As a facilitator of the child and family team, the Care Coordinator assesses the family’s strengths and needs and develops an initial and then ongoing individualized plan of care that reflects the culture values and needs of the family
Coordinates regular family care coordination team meetings, collaborates with multiple community support services and authorizes services included in the plan of care
Ensures the Family’s experience, voice, access, and ownership of the plan is consistent with the wraparound model
Support FDTC-PIR participants in the context of their family team as they recover from substance use disorder and develop the skills and ability to safely parent their children
Interviews clients, collects and completes required documentation and determines eligibility for assistance based upon a variety of service regulations and procedures. Monitors and ensures contract and program compliance. This is a home-based service program and requires regular in-person contact with clients and team members
Provides basic needs counseling and problem-solving assistance and/or intense case management services for clients. Conducts assessment of client needs. Gathers family story, develop family team plan, monitors progress and follow-up. Provides social services related to crisis intervention
Coordinates with other service agencies, organizations, and vendors to provide appropriate services and information for clients as well as referrals. Serves as an advocate on behalf of client. Makes appropriate referrals to other social services. Interprets eligibility requirements, policy and procedures to clients, other agencies, vendors and governmental entities
Assists in contract development and monitoring
Plans, organizes and participates in special projects, including outreach efforts, public speaking and special activities for client. Provides education and training to clients/community
May provide short and long-term case management, crisis intervention and service coordination to ensure maximum self-sufficiency and non-institutional living. May provide short-term independent living skills assistance and training
May assist clients in obtaining vocational and employment services
Performs other job-related duties as assigned
Qualification
Required
Bachelor's degree in one of the Social or Behavioral Sciences AND two (2) years experience in social services related work
Any combination of education and experience that has been achieved and is equivalent to the stated education and experience and required knowledge, skills, and abilities sufficient to successfully perform the duties and responsibilities of this job
Valid Texas Driver's License
Knowledge of: Policies, practices, procedures and terminology of assigned function
Knowledge of: Federal, State and Local program laws and requirements
Knowledge of: Modern office practices, procedures and methods
Knowledge of: Computer equipment to include word processing, spreadsheets, databases and a variety of software packages
May be required to develop knowledge of and adhere to federal and state laws requiring the confidential handling of certain health information

Preferred
Bilingual, Fluent in English and Spanish
Five (5) years of experience in social services related work
Five (5) years of relevant wraparound/family team facilitation experience
Three (3) years of relevant child welfare experience
Experience working with non-traditional families. Must be non-judgmental and feel comfortable working the families' homes or being creative in figuring out a community place to meet and work with clients
Experience conducting home visits
Strong familiarity with community resources
Experience providing intensive case management services
Experience working 1:1 with individuals and families in crisis
Experience with MS Office, database systems, internet, email, and text messaging
