Senior Program Manager - Residential DDABI

Responsibilities

Consult with participants prior to life plan meetings to determine participants’ goals, interests, satisfaction with services, etc
Participate in life plan meetings to assist in developing life plans; follow up with care managers to assure accurate plan goals and timely plan completion
Complete Individual Protective Oversight Plans (IPOPs) minimally twice a year, or more if participants’ needs change
Monitor and ensure completion of staff assignments and change of shift protocol
Monitor and ensure correct staff administration of medications and documentation, per Agency policy
Work collaboratively with IRA nursing staff to ensure proper medical oversight of participants, and to ensure that all medical and/or specialist recommendations are followed; ensure participants’ attendance to medical appointments and provide support, as needed
Observe staff and review staff documentation to ensure person center services are provided; and to ensure that life/staff action plans, IPOPs, dietary guidelines, Plan of Nursing Services (PONS), and behavior support plans, are implemented correctly
Implement a system to communicate with participants and families/advocates in order to monitor their satisfaction with services and to ensure they feel services are implemented as described in the service plan
Ensure that medication procedures, behavioral support interventions and emergency safety procedures, are developed, practiced, and revised as needed, including fire drills
Responsible for the supervision of assigned Assistant Managers and Direct Support Professionals (DSPs)
Provide ongoing direction, support, and regular one on one supervision to staff
Conduct performance evaluations of staff-initial, 6 month, and annual; document performance concerns and follow-up with disciplinary action, if necessary
Manage staff schedules to ensure adequate shift coverage
Share on-call responsibilities
Monitor and ensure participant safety in the IRA, in the community, and during transports
Report participant incidents per Agency policy, including reporting to the NYS Justice Center, as needed
Monitor the implementation of Plans of Corrective Action (POCA)
Provide additional training and support to new Program Managers, as identified by supervisor
Take an active role in the training of new staff, i.e. extended orientation, supervisory training, or other specialty trainings
Act as the Program Manager for an IRA that has a long-term Program Manager vacancy, until said vacancy is filled, as directed by supervisor
Understand the assigned IRA budget and assist the Director in its development
Ensure all documentation required to support Medicaid billing is current and maintained in participants’ records
Provide person-centered, quality, services
Follow Agency standards and procedures
Maintain compliance with applicable program rules and regulations
Contribute to department and Agency quality improvement initiatives
Perform Other Duties as Assigned

Required

Associate degree in a human services field, plus one years' experience working with people with disabilities, required
High school diploma/GED plus two years' experience working with people with disabilities, required
Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs
Must respect confidentiality
Must be available for occasional early morning, evening, or weekend functions, and share on-call responsibilities
Must be able and willing to demonstrate direct care duties, including personal assistance
Must be able to lift 75lbs
