Intake and Discharge Coordinator, Recuperative Care - 10751
The Colorado Coalition for the Homeless is dedicated to preventing homelessness and providing solutions for at-risk individuals and families. The Intake/Discharge Coordinator will facilitate the intake and discharge processes for participants in Recuperative Care programs, ensuring a supportive environment for healing and recovery.

Responsibilities
Acts as the point person for the day-to day operations of Recuperative Care Intake and Discharge System; ensures smooth coordination of daily intake process, receiving programs and property management
Understands contractual requirements and works with Operations Manager to ensure Intake/Discharge Operations are meeting all targets
Identifies goals and strategies to better conduct integrated-care procedures, better serving the needs of homeless individuals
Frequently utilizes spreadsheets and other data-tracking and analysis tools and applications
Provides ongoing program and navigation services in conjunction with integrated care team
Performs other duties as assigned
Qualification
Required
Valid driver's license required
Sound critical thinking and decision-making skills
Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with underserved populations
Ability to take initiative to implement projects and pursue goals with minimal supervision
Microsoft Excel or other spreadsheet program experience is a requirement
Excellent organization skills and willingness to work flexibly under pressure
Preferred
Bachelor's degree or relevant experience in a related field is preferred
Experience serving homeless community or public health sector preferred
Experience working with integrated medical team is preferred
