Clinical Pharmacist Specialist (Cardiology)

Responsibilities
Participates in Professional Practice Evaluation Activities required to maintain an Advanced Scope of Practice
Performs other administrative duties as assigned by the Chief of Pharmacy Service, Associate Chief for Clinical Pharmacy Services and the Chief Medical Officer
Core functions include face-to-face, telephone and telehealth encounters with referred patients, for comprehensive medication management in practice areas identified in the individual scope of practice
Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas
Conducts patient interviews to determine a medication history and additional information regarding disease state/drug therapy when evaluating a clinical situation. When necessary, educates patients on the proper use and expected effects of their medications
Accesses appropriate patient records for proper evaluations of clinical situations and therapeutic decision making
Develops individualized therapeutic plans utilizing the most efficacious, least toxic, and most economical pharmacological treatment modalities available, and communicates interventions verbally or in writing to pertinent members of the healthcare team
Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters
Assumes clinical responsibility for the Cardiology area of practice (both inpatient and clinic based services). This will include maintaining awareness and understanding of ongoing Pharmacy programs, as well as assuring competency to perform under his/her approved Scope of Practice
Promotes the use of formulary agents whenever possible. In accordance with the non-formulary drug request consult process, provides information on non-formulary issues to health care providers. Adjudicates non-formulary consults as assigned and includes appropriate and complete documentation
Serves as a preceptor to students of Affiliated Schools of Pharmacy and to pharmacy residents
Assists in the training of new pharmacy staff
Performs in-service training as scheduled
Independently secures funding and conducts clinical research within the scope of Primary Care

Education-Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Experience, must have 1 year of experience equivalent to the next lower grade level
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
Preferred
Successful completion of a PGY-1 Pharmacy Residency and PGY-2 Cardiology Pharmacy Residency
