Responsibilities
Round regularly and manage wound care for patients at assigned skilled nursing facilities (SNFs)
Collaborate with wound care nursing staff to assess and treat residents with wounds
Deliver high-quality, patient-centered, comprehensive wound care services
Complete timely and accurate documentation for all patient encounters
Be available for phone and secure messaging consultations with facility nursing staff
Document care in Theoria’s ChartEasy™ EHR and interface with facility EHRs (PointClickCare, MatrixCare, American HealthTech)
Qualification
Required
Wound Care Nurse Practitioner or Physician Assistant certification
Ability to manage wound care for patients at assigned skilled nursing facilities (SNFs)
Experience collaborating with nursing staff to assess and treat residents with wounds
Ability to deliver high-quality, patient-centered, comprehensive wound care services
Timely and accurate documentation for all patient encounters
Availability for phone and secure messaging consultations with facility nursing staff
Ability to document care in Theoria's ChartEasy™ EHR and interface with facility EHRs (PointClickCare, MatrixCare, American HealthTech)
Ability to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation
