TapestryHealth is a leading technology-enabled provider group dedicated to transforming medical care for post-acute communities. In this contractor role, the MD will be responsible for establishing the TapestryHealth patient relationship for our chronic care management services.

Responsibilities

Responsible for establishing the TapestryHealth patient relationship for our chronic care management services

Availability 10+ weekdays per month to conduct approximately 375 clinical visits onsite and remotely. These are brief, educational consults simply advising patients of the services they are enrolled in

Document patient encounters accurately and thoroughly within our platform at time of visit

Provide end of day report to program administrators to track progress for completion of visits

Qualified MD/DO

Active, unrestricted, Medical License(s) in California and multiple other states

Excellent clinical judgment and decision-making skills

Must have reliable transportation to travel to assigned buildings

Commitment to providing patient centered care and improving health outcomes

Familiarity with electronic health record (EHR) systems and healthcare technology

Ability to travel within 1-2 hours of assigned region
