Responsibilities

Responsible for providing a supportive, home-like environment for individuals we serve, in accordance with the principles of normalization, the agency’s mission, and all Federal, State, and County regulations
Responsible for staffing and supervision of Direct Care and Home Supervisor
Develops, monitors, and coordinates IP’s to allow individuals we serve to develop to their fullest potential in areas of self-direction, independent living, self-care, self-sufficiency, language learning and mobility
Manages the home and ensures programming is in adherence to all Federal, State and local guidelines
Ensures the safety and health needs of the individuals we serve
Achieves performance outcomes
Conducts audits of services as scheduled
Ensures the rights and welfare of the individuals we serve are guaranteed
Responsible for writing and managing the Plan of Correction on any citation received during licensure, Medicaid, in conjunction with the ICF Director
Responsible for completing 90-day and annual employee appraisals on a regular and timely basis
Conducts monthly staff meetings
Attends monthly manager’s meeting
Participates in the recruitment of staff and interviews, orients, trains and monitors job performance of Direct Care and Home Supervisor
Provide training in person centered programming to Direct Care and Home Supervisor
Oversees the home supervisor to ensure equitable coverage based on staff competence, experience and client acuity
Attends on-going scheduled training sessions, in-service programs, staff meetings, Human Rights, IHP/ISP’s, meetings at the workshop, and schedules staff to attend the same
Provides 24 hour contact with the home through a pager/cell phone
Ensures the home stays within its budget in all areas (programming, staffing, groceries, supplies, etc.)
Is accountable for staying within the threshold of the Quality Indicators (care giver satisfaction, family satisfaction, licensure and Medicaid survey’s, resident satisfaction, abuse/neglect substantiations, and other audit items)
Acts as a model of gentleness and non-violence
Is required to complete training to the Gentle mentor level within the first two years of employment
Adhere to other training requirements as necessary
Assumes responsibility for working with and supervising consultants (Speech therapist, Dietician, OT, PT, Psychologist) and volunteers
Is accountable for reporting, documenting, MUI’s and UI’s in a timely manner as well as developing and monitoring corrective action plans
Investigates MUI’s as directed and within timeline required
Write, review, and monitor annual assessments and IHP programs
Reviews monthly progress reports and updates PIPS
Schedules and conducts annual IHP meeting
Assures the implementation of quality programming in home to meet the standards of active treatment as well as the needs of the individuals we serve in the home and workshop
Conducts audits at the workshops at least every other week
Actively seeks resources (generic/adaptive) to assist in provisions of programming
Maintains communications with other agencies, resource persons, and consultants, as needed, to ensure that methodologies used between disciplines do not contradict one another
Establishes regular communications with ICF/IID Director concerning information regarding programs, resident concerns, and issues with the home or community
Monitors all inquiries by family members, advocates, volunteers, and community members
Communicates with family members as necessary
Actively participates in the establishment of a positive community presence
Monitors individuals’ health needs
Maintains habilitation records for each individual
Responsible for scheduling community outings
Reviews admission information on prospective candidates for admission to Graceworks Enhanced Living
Coordinates admission/transfer/discharge paperwork for persons served, while following all regulations
Interacts and develops relationships with Federal, State, and local licensing and regulatory bodies for the benefit of our clients and Graceworks Enhanced Living
Must be available and present during all surveys
Responsible for following vacancy procedure for all admissions
Meets thresholds for census
Oversees all volunteers including regular progress meetings, annual evaluations, schedule and report volunteer hours monthly
Works in unison with the Safety/Risk Analyst to ensure the safety of the homes, clients, and employee
Trains staff on workers compensation prevention and proper techniques
Works in unison with Human Resources on employee discipline and employee retention issues
Adhere to all policies and procedures of the organization and all federal and state laws regarding the privacy of protected health information
Adhere to all policies and procedures of the organization regarding the use of technology and technology-based information
Completes month end reports in care tracker timely
Enters and edits current & existing data for assigned residents
Responsible for welcoming and on-boarding new employees
Other duties as assigned

Required

Bachelor's degree from an accredited program in Special Education or related field
Medication Passing, CPR and first aid certification
Valid driver's license and good driving record
At least two years DD programmatic experience with two to three years supervisory experience
Experience in working with dually-diagnosed persons or persons with behavioral problems is a plus

Preferred

QIPD and IDD experience preferred
