BH Care Management Coordinator Team Lead

The BH Care Management Coordinator Team Lead will provide direct patient care services and coordinate the care of behavioral health patients while leading a team to promote positive treatment outcomes.

Responsibilities

Performs leader functions, i.e., assessment of qualifications, skills and competencies, provides orientation and training, and provides input into performance review and hiring selections
Provides clients with self-management skills and educational information ensuring full participation in their own treatment and recovery
Coordinates the care of an identified behavioral health population throughout the healthcare continuum
Acts as part of a multidisciplinary team and contributes to the development of a comprehensive plan of care based on a systematic assessment of the physiological and psychosocial needs of the patient
Assists the treatment team in recognizing, treating and managing mental health and psychosocial issues
Communicates and collaborates with patient and/or family to ensure understanding of treatment plan
Monitors patient progress towards established goals and evaluates and modifies plans of care as needed
Documents treatment goals and plans in the electronic medical record of which includes all assessments and all relevant clinical documentation
Utilizes behavioral health expertise to promote change and improve behavioral and physical health conditions
Provides care-management skills and knowledge of local resources for outside referrals
Advises the care team about suitability of clients’ settings, i.e. other mental health facilities, specialties, etc
Assists in the process for any recommendations that need required authorizations prior to arranging for treatment
Maintains a positive, collaborative relationship with other departments within the hospital district

Required

Master's Degree in Counseling or Social Work
One plus year's experience working in a team-oriented, fast paced, collaborative, behavioral health environment
2 plus years professional social work experience working with behavioral health/mental health patients in a healthcare, therapeutic, physician and/or other clinical practice setting
At least one year of prior team lead/supervisory/management experience
Basic knowledge of Microsoft Office Suite to include Word, Excel, and Outlook
Must have current licensure from Texas in one of the following: Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) from the Texas State Board of Social Work Examiners or the Texas State Board of Examiners of Professional Counselors
Certification in Non-Aggressive Crisis Management; must be obtained while in orientation and maintained throughout employment

Preferred

1 plus years of experience leading, supervising and/or managing experience
2 plus years of prior supervisory/management experience
Licensed Clinical Social Worker (LCSW)
