MUSCP-Program Coordinator-Retina Specialization-Department of Ophthalmology
The Medical University of South Carolina is seeking a dedicated and compassionate Patient Coordinator for their Ophthalmology Department. This role involves working with retina subspecialists to provide comprehensive care coordination for complex eye patients, facilitating communication, education, and services to enhance patient adherence and health outcomes.

Responsibilities
Help develop individualized care plans based on assessed needs, including medical management
Teach patients about medications
Support the physician by pre-scribing and live scribing in clinic
Implement communication strategies for persons eye disease/blindness and their caregivers
Facilitate referrals by obtaining clinical information and ensure the information is properly processed
Review upcoming appointments in Epic to ensure necessary services are arranged before the appointment
Monitor pre-authorizations for expensive injectable medications. Assist patient with assistance programs as needed
Ensure all testing and referrals are complete and necessary items addressed with the provider
Review all patient test results, such as imaging and lab work, with providers and execute the care plans discussed
Coordinate with referring physicians' offices to meet the needs of both patients and physicians
Provide comprehensive education on symptoms, disease progression, treatment options, coping strategies, and caregiver support to patients and families, resulting in increased understanding and improved outcomes
Help create clinic efficiencies such as improved clinic flow
Serve as primary clinical contact for Ophthalmology patients enrolled in the patient navigation program and their caregivers regarding scheduling of testing and appointments
Serve as the liaison between the Storm Eye clinical team and referring physicians
Provide emotional support tailored to the patient's clinical situation
Communicate with external stakeholders to ensure patients have access to individualized community support services
Qualification
Required
Minimum of an Associate's degree and one year Ophthalmologic experience
High school diploma and two years of clinical experience, one year must be Ophthalmology-specific
A Bachelor's degree and one year work experience
High school diploma (or equivalent) and 2 to 4 years work experience
Preferred
Ophthalmic technician certification
