Responsibilities

Contributes to View Point Health’s mission, vision, and program philosophies by way of work product and professional behavior with both internal and external consumers
Adheres to licensure requirements and regulations as set forth by regulatory agencies
Participate actively in agency quality assurance activities including performance and outcomes review, case record review, stakeholder satisfaction review, incident review, and improvement planning. (DDP role)
Demonstrates an awareness, appreciation and respect for diverse cultures and individual differences between clients and fellow employees
Adheres to all current federal & state laws, as well as View Point Health’s Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability & Accountability Act (HIPAA)
Ensure that all Residential Licensing and Contract Standards, other standards and View Point Health’s policies and procedures are implemented
Reports immediately any suspected incidents of abuse or neglect to his/her immediate supervisor and the Corporate Compliance Office
Establish relationships and communicates with residents, support workers, family members, and staff
Identifies and seeks resolution for potential problems arising with staff or other parties involved
Attending staff / agency meetings
Attends training, conferences, workshops, seminars as needed to remain current with DD practices
Reports to the Assistant Director of Developmental Disability Services regarding support services, personnel and fiscal issues, etc
Assists the Assistant Director of Developmental Disability Services in short and long-term planning for the program
Responds to emergency and crisis situations
Assists with ensuring program policies and procedures are up to date and available for staff review
Complete and document facility inspections and generate work orders for repairs when necessary
Prepares for any off-site travel activities (i.e. needed items, menus/food, etc.)
Provides leadership and direction to supervisors and direct care professionals. Acts as a role model and mentor
Listen to staff concerns and work with them to resolve
Assists with staff recruitment and hiring
Facilitate orientation and initial training of new staff
Provides ongoing training through on-the-job training and curriculum-based training
Oversees work schedules for staff to ensure adequate supervision of residents
Assists in the review of staff time sheets and vacation requests
Assists with completing staff performance evaluations, employee corrective actions, performance plans and terminations
Participates in intake staffing
Develops Recreation Calendar as applicable for the program
Reviews and approves Incident Reports and provides feedback to staff when necessary
Attend program activities held off site as necessary
Assists with community service projects
Intervenes in crisis situations involving residents when necessary
Arrange transportation for residents as necessary
Attend court hearings when necessary
Overseeing the services and support provided to participants for general guidance to the provider agency in areas of compliance and quality improvement
Ensure that the support provided is within the scope of the agency’s service enrollment and experience to ensure effective delivery
Assuring that the services address the participant’s needs and adhere to the application of person-centered values, choice and participant’s rights
Providing, arranging or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goal(s) and objectives
Recommend other needed services/supports or changes to the delivery model using a continuous quality improvement approach
Providing consultation to the provider agency in ISP implementation strategies that are specific, measurable, achievable, relevant, realistic and time limited in order to meet the needs and personal goal(s) of the participant
Assess areas of risks either individually or overall risks to individuals supported through agency practice, policy or lack of policy or procedures/protocols. Providing risk mitigation strategies to the provider agency
Reviewing that functional assessments are in place to support formulation of the participant’s plan for delivery of all waiver services that include: The Health Risk Screening Tool, The Supports Intensity Scale, Functional Behavioral Evaluation, Others (E.g., Nursing, OT, PT etc.) as needed or required
Oversee high intensity services if applicable that address health and safety risks for the participants that include: The implementation and effectiveness of Behavior Support Plans, The implementation and effectiveness of the Participant’s Crisis Plan; and Identifying ongoing supports as needed (medical and /or behavioral) in collaboration with agency staff, staff of other agencies providing support to the participants mutually served or other members of the healthcare team

Required

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or Two (2) years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or equivalent position
Must be able to lift 20 pounds
Requires long periods of sitting/standing
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy
Candidates for selection must pass a criminal background check (including fingerprinting)
Must pass a pre-employment drug screen and subject to random drug screens

Preferred

Supervisory or lead worker experience in a group home or similar setting
Experience working with individuals with Developmental Disabilities
Proficient use of Microsoft Application (Excel, Powerpoint)
