The Social Services Designee supports residents and families throughout the admission and discharge process by managing referrals, screenings, tours, approvals, and documentation while also marketing facility services and building relationships with referral sources.

Responsibilities
Managing referrals, screenings, tours, approvals, and documentation
Marketing facility services and building relationships with referral sources
Clarifying legal documents and assisting with emotional and psychosocial needs
Helping residents navigate advanced directives, financial aid applications, and transitions within the facility
Conducting social assessments, mental health screenings, and required MDS/CAA documentation to develop individualized care plans
Maintaining accurate records and ensuring regulatory compliance
Upholding professional communication and confidentiality standards
Participating in quality improvement efforts and contributing to policy development to support residents’ rights and overall well-being
Qualification
Required
Must recognize and understand the psychosocial and emotional needs of residents
Experience in healthcare, skilled nursing facility or assisted living community
Must have strong skills in communication, organization, management, and leadership
Must be able to perform essential functions of the position with or without reasonable accommodation
Must qualify for employment after a criminal background study, per guidelines of the Minnesota Department of Human Services
