The WG Bill Hefner Salisbury VA Medical Center is part of the Department of Veterans Affairs, dedicated to providing healthcare services to veterans. Responsibilities

Oversees the direction and completion of work by subordinates to ensure timeliness, accuracy, and completeness
Approves or disapproves leave requests, formulates work schedules, performs timekeeping functions, and ensures appropriate staffing for the work of the section
Assesses workload trends and needs and adjusts staffing requirements as needed to maintain access to care
Performs CPS duties as needed based on daily staffing needs
Develops and maintains systems to monitor the performance of staff activities
Ensures performance standards are maintained, staff orientation needs are designed and implemented, and clinical care reviews are performed
Maintains effective intra-department relations and inter-department relations to accomplish the facility missions and goals
Develops effective communication with AFGE and is knowledgeable on the master and local agreement
Assures security over pharmacy areas and provides an immediate report to management in event of inconsistencies
Assures clinical services within pharmacy service comply with regulatory and accreditation body standards, best practices, and coincide with the overall mission of the Salisbury VA Health Care System
Treats patients, team members, other facility employees and visitors with respect
Responds to the needs of the Veteran and staff in a courteous and timely manner
Demonstrates appropriate and proactive interpersonal skills to achieve the goals of the service
Acts as a team member and always supports efforts of both the pharmacy and clinical pharmacy specialist team
Demonstrates an ability to work under a variety of circumstances to maximize group productivity and quality
Core functions include face-to-face and virtual patient care (e.g., telephone visits, tele-health, and other virtual modalities) with PACT team patients, for comprehensive medication management in practice areas identified in their individual scope of practice
Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in covering areas
Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy
Follows-up with patients on lab or test results to discuss the plan of therapy, i.e., changes in medication therapy, monitoring, and additional testing requirements
Clinical pharmacy specialists will determine if a referral to the physician is necessary when disease progression occurs, or adverse drug events require treatment interventions outside of the CPS scope of practice
Identify patients who may benefit from comprehensive Medication management through population management reports and during team huddles
Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required
Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed
Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures

Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
Knowledge of Professional Pharmacy Practices
Residency and Fellowship Training: Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis
Quality of Experience: Qualifying experience must be at a level comparable to pharmacy experience at the next lower level
Ability to effectively supervise subordinate staff
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards pertaining to pharmacy
Skill in managing people or programs

Preferred

Candidates for the PACT Clinical Pharmacist Practitioner Supervisor position should possess the skills with specific experience in Ambulatory or Primary Care setting
Candidates should have received experience in a current or previous job (including residency training) that includes direct patient care and comprehensive medication management responsibilities
Preferred candidates will have combination of the following: PGY1 Residency, PGY2 specialty residency, and/or board certifications (BPS); or equivalent training and experience providing direct patient care as a pharmacist provider
