Responsibilities

Daily Data Management for Compliance
Maintains and monitors inpatient rehab data to ensure compliance with missed minutes reporting requirements
Sends out daily reporting to unit staff to ensure maximum reimbursement
Patient Care Conference Scheduling
Coordinates and schedules required patient care conferences on 4WH, ensuring timely communication and collaboration among interdisciplinary teams
Facilitates continuity of care for patients and communication within the care teams
Referral Management for Prosthetics and Orthotics Clinic
Contributes to patient referral workflows, ensuring timely referral processing, flag for prior authorization needs and scheduling for specialized services
Enhances patient access and satisfaction through efficient referral management
Quality Improvement (QI) Data Oversight
Supports QI-related data for both inpatient and outpatient services, chart reviewing, supporting A2E goals and departmental performance metrics
Provides consistent and accurate data for analysis for leadership supported improvement initiatives
Credentialing and Compliance for Staff
Oversees credentialing, compliance and onboarding requirements for all employee hires, transfers and separations within Rehab Services
Ensures adherence to organizational and regulatory standards, reducing risk and maintaining operational integrity

Required

High school graduation or equivalent AND two years of experience in the program specialty
