The VA Eastern Kansas Health Care System is seeking a Care Coordination and Integrated Case Management (CCICM) RN Program Coordinator. The role involves providing a full range of nursing services, administering therapies, and promoting discharge planning while engaging with patients and their families.

Responsibilities

Provides full range of nursing services utilizing the nursing process
Administers prescribed therapies using nursing judgement
Promotes discharge planning and follow up care as needed
Listens to and includes patient and family in plan of care
Provide feedback and patient education
Communicates with Veterans on their health care options including local VA, other VA medical centers and community-based eligibility
Works closely with other VA Medical Centers, contracted facilities, and community facilities to remove obstacles that would impede the Veteran from utilizing the services of other VA Medical Centers
Educates providers on medical management processes
Provides clinical knowledge and acts as a clinical resource to non-clinical multidisciplinary staff

Required

U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy
All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment
Selective Service Registration is required for males born after 12/31/1959
Subject to background/security investigation
Selected applicants will be required to complete an online onboarding process
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
English Language Proficiency
Graduate of a school of professional nursing approved by the appropriate accrediting agency
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth of the United States, or the District of Columbia
