Responsibilities
Provide inpatient care coordination services for Mental Health patients to facilitate optimal patient outcomes
Support the delivery of effective and efficient care
Improve continuity of care and efficient use of resources throughout the episode of care
Collaborate with the Care Team including but not limited to: Attending/Specialty Physician(s), Social Work, Payors, Utilization Review, and Nursing
Ensure that an effective and timely treatment plan is acted upon
Implement an appropriate discharge plan with smooth transitions in levels of care
Mine and analyze clinical and/or operational data and develop reporting that is critical to operational leadership at the site and organizational level to assure effective program delivery and process improvement
Perform related duties as assigned
Qualification
Required
RN with a Bachelors Degree and licensed as a registered nurse by the State of Minnesota
5 years of increasing responsibility in acute care hospital setting including data analysis and reporting, project management and program operations
CPR for the professional rescuer or BLS for healthcare providers
Preferred
Minimum 1-3 years clinical experience in case management or mental health setting; experience in project management or discharge planning, and/or quality improvement preferred
Masters degree in mental health field or expected graduation within one year, PLUS 1-3 years direct experience in case/program management
