Responsibilities
Serve as the community expert for the Blue Shield of California Promise Health Plan
Create meaningful relationships with community-based organizations
Participate in community events and act as the health plan liaison
Assist members with complex health and/or social needs by assessing their immediate needs and addressing barriers to care, including care coordination and linkages to health plan services and community resources
Qualification
Required
Experience in community engagement and outreach
Strong understanding of healthcare access and health equity
Ability to build and maintain relationships with community-based organizations
Experience in assessing health and social needs of individuals
Knowledge of care coordination and linkages to health plan services
Excellent communication and interpersonal skills
Ability to participate in community events
Willingness to travel frequently within San Diego County
