Early Intervention Long Island Service Coordinator
Autism Care Partners is seeking dedicated professionals to join their Early Intervention team. The Service Coordinator plays a vital role in helping families of children with developmental delays navigate the Early Intervention system by coordinating services and providing case management.

Responsibilities
Communicate and assess families concerns, priorities and resources, strengths and needs
Ensure services are provided as per the Individual Family Service Plan (IFSP)
Assist families to access community resources and services
Complete notes after each contact made
Submit paperwork on a timely basis
Communicate with child’s EI team including providers, outside agencies, municipality and ACP-EI staff
Qualification
Required
Degree in a social services field and/or experience with young children with developmental delays
Effective written and verbal communication skills
Well Organized and effective time management skills
Must follow agency and municipality policy and procedures
Submit all documentation on a timely basis
Ability to work independently and be an effective team member
Available for daytime meetings
Preferred
Knowledge of child development a plus
Spanish Speaker
