They are seeking a Family or Internal Medicine Physician to provide comprehensive, patient-centered care in a full-time outpatient primary care role.

Responsibilities
Serve as the primary care provider for a defined patient panel within a Patient-Centered Care Model
Conduct outpatient visits, manage ongoing communication, coordinate safe transitions of care, and refer to appropriate specialties
Assess health risks, promote disease prevention, and practice evidence-based medicine with support from integrated care teams
Qualification
Required
MD/DO with completion of an accredited Family Medicine or Internal Medicine residency
Active Massachusetts medical license
Board Certified (FM or IM)
DEA and Massachusetts Controlled Substance Registration (MCSR)
Current BLS certification
Preferred
Experience with a community health center population and practice setting preferred
