Family Guidance Centers, Inc. (FGC) is a not-for-profit behavioral healthcare organization focused on treating and preventing substance use disorders. They are seeking interns from Governor's State University to assist in various responsibilities including case management, community outreach, and residential coordination to support individuals in their recovery journey.

Responsibilities
Reviews assessed case management needs as expressed by clients and as indicated on the client treatment plan
Assists with monitoring patient engagement and communicates with the treatment team regarding patient attendance, participation, and any barriers or needs to support treatment involvement
Facilitates Orientation with patients to review FGC OMT handbook and tracks all newly admitted patients to ensure orientation is completed within 21 days of admission
Processes all newly admitted patients from referring detox unit into level 1 OMT (including scanning all documents daily)
Coordinates with detox staff via email from referring hospital with appropriate paperwork as required; communicates with referring hospital units to ensure a smooth transition (Nursing staff and Toxicology department)
Focuses on patient outreach and crisis response services for all FGC sites
Engages in outreach (mostly via telehealth) to individuals' no longer in treatment, including no show for admissions, to encourage re-engagement into treatment as well as determine and assist with any needs
Provides crisis response services, mostly through telehealth, to individuals who have left FGC to help resolve crisis situations
Coordinate with various FGC team members (such as Immediate Access to Care Team at 310 and Wabash) to schedule an individual's admission process as well as other team members from all FGC outpatient sites
Provide patients or family members with information about addiction issues and about available services or programs, making appropriate referrals when necessary
Directly engage with individuals experiencing substance use problems through street outreach and outreach/engagement services provided on-site at homeless shelters and drop-in centers in order to ensure connection to substance use and mental health treatment
As part of the community outreach team, the outreach team specialist provides information about treatment options, including medication- assisted recovery (MAR) services available in the community
Communicates daily with FGC medical staff with FGC's managers/admission staff at all program locations, to determine daily program census needs
Develops formal relationships with the area behavioral healthcare providers, hospitals, clinics, and other social service agencies(i.e. DCFS providers, DHS local offices)to establish referral relationships and to effective process
Presents FGC program descriptions and eligibility requirements to community leaders who represent local social service, church and political organizations
Professionally represents FGC at community organization events, seminars and conferences
Support treatment plans by reporting client observations (at a minimum twice per shift and subject to change depending on need) - on appropriate forms - and supporting decisions concerning therapy, discipline, and privileges
Assists case manager/counselor with client activities by implementing scheduled and/or requested activities
Works cooperatively with members of the nursing and medical staff to monitor clients' vital signs, observe urine drops, monitor clients, and chart all pertinent data
Monitor self-administration of medications by clients and document in pertinent logs according to regulations
Performs room and facility checks, at varying intervals, to ensure client's comfort, safety, and well-being

Required
