Case Manager - Central Scattered Site - 10765

Colorado Coalition for the Homeless is dedicated to preventing homelessness and creating lasting solutions for at-risk individuals and families. The Case Manager will provide community resources, conduct case management, and assist clients in maintaining their housing stability.

Responsibilities

Assesses strengths and needs of all new residents
Develops initial treatment/service plans for all residents and update them as required (every 6 months at a minimum)
Develops interventions or linkages to address resident needs
Provides hands-on assistance and/or training to residents in order to support them in maintaining their housing in a safe and sanitary manner
Ensures that program benefits the greatest number of individuals possible by collaborating with the intake team regarding program census
Ensures that each resident is receiving the support and services they require to maintain their housing and support recovery
Assists residents who desire to move on to a more independent level of housing
Assists property management staff in problem-solving regarding issues involving participants of the program
Creates social and psycho-educational opportunities for residents and maintain a monthly events calendar
Ensures all verification paperwork is completed as required by law and/or funding sources
Provides crisis intervention as required and in collaboration with clinical staff and community partners
Assists residents in problem-solving issues which may threaten their housing
Serves as a liaison between clinical staff and property management staff
Serves as liaison between resident and other service providers, including property management staff as necessary and appropriate, especially when necessary to advocate for clients’ housing stability
Assists in transportation of residents
Provides coverage for other case managers as necessary
Performs other duties as assigned

Required

Effective interpersonal/assertive communications skills demonstrating a high degree of emotional intelligence
Knowledge of homeless issues and demonstrated sensitivity to underserved populations
Ability to listen effectively, synthesize and analyze information, and respond creatively to challenging issues
Skill in negotiating and mediating, particularly in sensitive situations
Ability to organize and prioritize multiple responsibilities simultaneously
Ability to ensure accuracy of data management and reporting
Ability to make sound decisions, using available information while maintaining appropriate confidentiality
Knowledge of local services and resources
General knowledge of mental illness and substance abuse including symptoms and treatment options
Ability to advocate for individuals in a fair and tactful manner
Valid driver's license, ability to pass automobile insurance carrier's motor vehicle record investigation, criminal background investigation and ability to drive a 15-passenger van
Experience and interest in working with disenfranchised persons
Must have driver's license

Preferred

Bachelor's degree in social service or related field preferred
Bilingual preferred but not required
