They are seeking a Residential Counselor to support group home residents in daily living activities and deliver high-quality services according to Individual Rehabilitation Plans (IRP).

Responsibilities

Support consumers in achieving personal care goals and addressing ongoing challenges
Educate consumers on medications, track logs, and conduct monthly audits
Coordinate services based on individual needs
Transports consumers using the company van as needed
Deliver high-quality care in line with Individual Rehabilitation Plans (IRP)
Accurately document services within required timeframes using electronic health records

Qualification

Bachelor’s Degree in mental healthVerbal communicationWritten communicationMicrosoft OfficeValid driver’s license

Required

Bachelor's Degree in mental health, psychology, sociology, or social work, human services or a related field from an accredited college or university
A combination of one (1) or more years' of college, plus related life or work experience equal to four (4) years
High school diploma or GED plus four (4) years of related work or life experience
Possess a valid driver's license & acceptable driving record
Strong verbal and written communication skills
Proficiency in Microsoft Office Suite, including Outlook, Excel, and Word
