TCM Analyst -Children Specialist

Responsibilities

Provide active daily support to peers in the TCM Unit
Advisor to the agencies on agency specific activities related to the Targeted Case Management rules and regulations as determined by DPW and adopted by the Philadelphia Office of Mental Health
Provide TCMs, Supervisors and Directors within our provider network with technical assistance and support with regard to identified problems and issues that may need to be addressed on an individual or agency basis
Provide agency specific, skill enhancement training and technical assistance through “modeling” techniques in their direct service environment
Provide technical assistance regarding the unified service plan and the complementary unified service planning process
Assist agencies in the acquisition of special types of assessments for clients, special service need configurations, service delivery case review sessions, team meeting observations or by special request of the agency
Attend training sessions to provide TCMs with the opportunity to provide feedback on systems issues and impact current information that will be relevant to their daily activities
Perform liaison functions to City and/or State departments as it pertains to TCM initiatives and functions, assuring adequate representation of the Philadelphia model of service provision and system expectations
Provides clinical support and guidance on outreach efforts and client specific interventions being provided by unit members
Coordinates all child related planning for TCM. Responsible for coordinating the Children's Supervisor meetings and the Basic Orientation Training for the children case managers
Participates on CBH children’s planning workgroup and works with a community committee with links to the schools
Serves as our liaison with the Centers of Excellence for Autism, as we relate to the CBH unit that oversees that project
Responsible for the coordination and writing of the program standards for the children’s BCM programs
Complete applications for and perform linkages to needed services, benefits, and entitlements
Other duties as assigned or delegated

Required

Working knowledge of the Philadelphia TCM system as well as the developing Behavioral Health Care components
Direct experience as a case manager and/or supervisor a major plus
Well versed in the DBHIDS Practice Guidelines, Philadelphia Mental Health system CSP/CAASP components - locations, types, accessibility, criteria, etc
A clear understanding of the TCM rules and regulations (both a county and state perspective/expectation) for service provision from the provider agencies
Ability to develop and execute trainings/technical assistance learning opportunities relevant/relative to case management (adult/child) initiatives
Clinical, assessment/evaluation skills particularly as it relates to TCM are required
Flexible, personable style, with strong verbal and written communication skills is recommended
Knowledge of computers (Microsoft Office applications) a plus
Minimum of a Bachelors Degree in the Health Care field
Five (5) years direct experience with TCM in the Philadelphia Mental Health System

Preferred

Masters degree preferred
Individuals with mental health lived experience are encouraged to apply
