They are seeking a Case Management Assistant to support RN case managers and social workers in facilitating patient discharges and providing administrative clerical support in Case Management functions.

Responsibilities
Coordinate with RN case manager and social worker for support needed. Communicate updates on progress throughout the day
Set up transportation via the Transfer Center and print discharge packets for post-acute care transfers
Use web-based referral system to manage post-acute care referrals
Prioritizes workload to ensure a timely contact and intervention with PAC providers
Monitors patients with post-acute care referrals and intervenes to expedite discharge
Communicate with patient and have the Patient Choice letter signed
Deliver the Import Medicare Message letter to the patient per facility policy
Qualification
Required
Medical knowledge and terminology is required
Minimum of one year experience in a health care setting is required
Possess the ability to work Monday - Friday, 8a - 4:30p and rotating weekends
Preferred
Associate Degree preferred
C.N.A license preferred
