Responsibilities
Support residents through psychosocial assessments and individualized care planning
Help residents and families navigate the transition to long-term and memory care
Complete MDS-related documentation and maintain accurate, compliant records
Advocate for resident rights, dignity, and self-determination
Coordinate discharge planning and community resources
Collaborate with nursing, administration, and the interdisciplinary team
Assist with survey readiness and regulatory compliance
Qualification
Required
Certified, licensed, or registered in the state of practice (required)
Bachelor's degree in Social Work or a related Human Services field
At least 1 year of social work experience in a healthcare or geriatric setting
Preferred
Skilled Nursing Facility experience preferred
Nursing experience in a SNF a plus
