Assistant Case Manager
RiseWell Community Services is an innovative, multi-service, community-based social wellness agency seeking a Part Time Assistant Case Manager to support the Outreach Services Department. The role involves assisting Case Managers in providing services to clients, helping them meet their basic needs through referrals and hands-on assistance.
Mental Health Care
Responsibilities
Assist Case Managers by providing selected services to ICM clients
Provide referrals, linkage, and hands-on assistance with obtaining food, employment, housing, and financial assistance
Qualification
Required
High School Diploma or equivalent
Individual must have personal experience with a mental health condition
Clean, valid New York State driver's license
Fingerprinting, criminal record check and all necessary clearances from the Justice Center
Preferred
Peer Certification preferred
