Assisting patients with scheduling of appointments while also identifying barriers to care
Improving access gaps which involves developing knowledge of the outpatient care setting and community services
Being accountable for successful tracking and linkage to care
Qualification
Required
Knowledge of theories, principles, and concepts normally acquired through completion of a Bachelor's degree in a Social Service field or equivalent experience in the field of HIV
One to two years of work related experience
Three to six months on the job training and orientation
