Licensed Mental Health Counselor- CPAN

Responsibilities

Informs providers and the larger community about the program using various means of communication (e.g., paper mail outs, emails, phone calls, faxes, virtual presentations, and in-person visits)
Invests time and effort in successfully and accurately navigating Lantana, Trayt, and other platforms
Enrolls providers in the CPAN-PeriPAN program using Lantana, Trayt, and other programs as needed
Maintains accurate and updated documentation of provider referrals, assessments, interventions, and recommendations, and ensures information is entered in needed spreadsheets/databases and health records within required timeframes
Completes initial evaluations for patients/families that will receive a direct consultation
Conducts screening assessments (such as GAD-7, PHQ-9, SCARED, etc.) and scoring of screening assessments to assist in the overall diagnostic determination of patients who will be seen for direct consults
Assesses for risk of suicide
Completes safety plans with patients as needed, when identified during an initial evaluation
Assists in the implementation of crisis intervention services, providing information about available options in the community for providers and families
Answers calls based on program operations and prioritizes tasks assigned
Offers guidance to providers on assessing the mental and behavioral health status and needs of patients
Identifies patients with urgent mental health conditions (e.g., suicidal, homicidal, psychotic, etc.) and makes appropriate connection to a child psychiatrist as needed
Identifies and facilitates coordination with an appropriate mental health professional (e.g., therapists, child psychiatrists) based on the level of care needed (e.g., medication management, therapy recommendations, available resources, etc.)
Offers information and resources to providers as needed for children, adolescents, and their parents/caregivers, including skill-based interventions (e.g., emotion regulation skills, behavioral parenting skills, distress tolerance skills, etc.)
Assists the provider in developing a behavioral plan based on information provided through the consultation with CPAN-PeriPAN
Updates monthly productivity reports and implements changes when needed
Maintains communication with providers and expert clinicians (Child Psychiatrists)
Facilitates expert clinician consultation (Child Psychiatry)
Maintains and updates professional knowledge and proficiency through continuing education, staff meetings, workshops and serving on special committees
Completes program projects in a timely manner by identifying and implementing new technology and resources
Efficiently and accurately prepares updated resource guides and handouts related to CPAN-PeriPAN programs
Attends meetings as needed
Collaborates with the data team and maintains all needed data
Constantly maintains a professional rapport and upholds TTUHSC values throughout all interactions

Required

Master's degree in psychology, counseling, social work, or related field
3 years of related experience
Licensed Professional Counselor (LPC)
Licensed Professional Counselor - Associate (LPC - A)
Licensed Clinical Social Worker (LCSW)
Licensed Master Social Worker (LMSW) must be working towards LCSW
Licensed Marriage and Family Therapist (LMFT)
Licensed Marriage and Family Therapist - Associate (LMFT-A)
Licensed Psychological Associate (LPA)

Preferred

Experience working in behavioral healthcare with the child, adolescent or perinatal population
