Responsibilities
Coordinate admissions from referral through intake
Support residents and families with social service needs and care planning
Communicate with hospitals, families, and interdisciplinary teams
Assist with discharge planning and transitions of care
Maintain accurate documentation and ensure regulatory compliance
Serve as a key point of contact for residents, families, and referral sources
Qualification
Required
Background in social services, admissions, or healthcare administration
Strong communication, organization, and multitasking skills
Compassionate, professional, and team-oriented approach
Preferred
Long-term care or skilled nursing experience preferred
Knowledge of insurance and discharge planning is a plus
