Responsibilities

Collaborate with primary case managers, individuals, families, and community waiver providers to complete annual service reassessments

Lead shared planning processes to ensure comprehensive support for clients

Document and ensure that all exceptional medical and behavioral support needs are included in the individual's annual person-centered plan

Maintain compliance with all relevant regulations and standards, including DMAS, DBHDS Licensing, Office of Human Rights, and DOJ Settlement Agreement requirements

Required

Bachelor's degree in education, social work, psychology, or human services related field is required

Minimum of two (2) years of experience completing and implementing Person Centered Individual Support Plans for clients receiving ID/DD Waiver Services is preferred

Candidates who are offered a position are required to undergo a background check and drug screen

This position requires transporting clients and driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years
