Behavioral Health Consultant (LCSW, Psychologist, LPCC)

Neighborhood Healthcare is a private, non-profit community health organization dedicated to improving the health and happiness of the communities it serves. The Behavioral Health Consultant will provide direct patient care for behavioral health services, including short-term psychotherapy and consultations with primary care providers, while adhering to an integrated care model.

Responsibilities

Provides short term targeted psychotherapy for problems identified by primary care providers
Performs initial and ongoing behavioral health assessment to develop patient care plans
Evaluates patients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical care
Provides brief psychotherapy to patients and families
Addresses and factors co-occurring disorders into patient’s care plan
Responds promptly and with caring actions to patients and employees
Provide crisis management and psychoeducation for patients and families
Conducts “warm hand offs” at medical provider requests
Acknowledges and honors patients’ psychosocial, spiritual, and cultural beliefs
Utilizes BHC staff protocols for referral, follow-up, and quality assurance
Consults with primary care providers about behavioral aspects of their patients’ medical treatment
Contributes to the success of the organization by participating in quality improvement activities
Maintains professional working relationships with all levels of staff, clients, and the public
Cooperates in accomplishing department goals and objectives
Participates in case conferences, trainings, and departmental or agency meetings, as directed
Follows BHC protocols, policies, and procedures
Provides personal information and participates in the credentialing process with managed care companies
Attends continuing education programs offered to upgrade professional skills and maintain licensing requirements
Documents legible and legally defensible medical records

Required

Doctorate degree in psychology with a valid CA Psychology License, or
Master's degree in social work with a valid CA LCSW License, or
Master's in degree in counseling with a valid CA LPCC License
Valid BLS certification from approved American Health Association provider required upon hire
Two year of pre/post licensure psychotherapy experience with children, adults, and/or adolescents required; child/adolescent experience required
Experience providing health psychotherapy services, such as smoking cessation, weight management, stress management, or work with chronically medically ill patients
Knowledgeable about and experience with displaying of normal signs of human development and ability to assess and provide age-appropriate care
Ability to work well in an interdisciplinary team within medical care
Ability to operate one or more word-processing, spreadsheet, and/or electronic medical records systems
Ability to do some charting simultaneously while in session
Excellent verbal and written communication skills, including superior composition, typing, legible handwriting, and proofreading skills
Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
Ability to successfully manage multiple tasks simultaneously
Excellent planning and organizational ability
Ability to work as part of a team as well as independently
Ability to work with highly confidential information in a professional and ethical manner
Ability to lift/carry 10 lbs/weight
Ability to stand for long periods of time
