Responsibilities

Maintains appropriate, applicable licenses, certificates and permits necessary or required
Proof of immunity to rubella and rubeola. Two doses of MMR vaccine or serological testing for immunity to rubella and rubeola required
Proof of varicella immunity be verbal or written history, or titers
Hepatitis B series initiation OR declination and/or testing to document serologic response
Two-step TB Skin test or Quantiferon testing within the last 12 months
Completion of flu shot within flu season (as indicated by state regulatory agency)
Proof of COVID vaccine or exemption documentation
ACLS and/or BLS Certification, if applicable
Completion of HIPAA Compliance Training
Training in Human Blood and Body Fluid Precautions and Infection Control

Required

You must be in the Military Active Duty, Guard or Reserve Component
Maintains appropriate, applicable licenses, certificates and permits necessary or required
