Providence Health Plan is dedicated to patient-focused, whole-person care. The Clinical Program Coordinator RN will provide care coordination services, manage disease management programs, and assist with case management for members with acute and complex health care needs.

Responsibilities

Provide care coordination services to Providence Health Plans (PHP) members

Manage disease management programs, including educating, motivating and empowering members to manage their disease

Conduct case management including: triage and referral, transition of care planning, end of life care planning, other acute and catastrophic case management

Offer care management services including nurse education, care coordination and general assistance with managing day to day functional needs

Assist with the management of member health plan benefits and offer assistance finding alternative services when benefits are exhausted
