Responsibilities

Supporting the enrollment and ongoing eligibility of participants in the CarePartners PACE program
Serves as a key liaison among potential and current participants, their families, local Department of Social Services (DSS), and internal care teams
Ensures timely and accurate processing of initial Medicaid applications and annual recertifications
Contributing to a seamless participant experience and regulatory compliance

Required

Valid NC Driver's License and reliable vehicle
Minimum 2 years of experience with Medicaid applications and eligibility processes, preferably in a healthcare or social services setting
Experience working with the frail elderly population
Associate Degree, or Bachelors Degree

Preferred

Bachelor's degree in Human Services or related field
Minimum 2 years of experience with Medicaid applications and eligibility processes, preferably in a healthcare or social services setting
Experience working with the frail elderly population
