The LPN will participate in patient assessment, care planning, and deliver patient-centered care in a collaborative environment with a focus on geriatric and palliative needs.

Responsibilities
Partner with providers, RNs, and allied health staff to deliver patient-centered care
Provide age-specific services, from newborn and pediatric rooming to adult and geriatric aftercare
Administer vaccines, medications, and treatments (injections, ear lavages, nebulizers, wound care, catheter care, suture/staple removal, vision/hearing screenings, fluoride varnish, etc.)
Manage patient communications via phone, online messages, and telehealth
Support preventative care and chronic condition management, reinforcing patient education
Independently manage nurse calendars, ensuring timely, accurate patient care
Contribute to quality initiatives by closing care gaps and improving outcomes
Qualification
Required
Licensed Practical Nurse (LPN) certification
Excellent communication skills
Ability to mentor and instruct
Experience in providing nursing care to assigned patients under the supervision of a registered nurse (RN)
Ability to collect data through observation and communicate information to assist in patient assessment and care planning
Experience in administering immunizations and medications
Experience in performing ear lavage, post-void residuals, intermittent and indwelling catheter care, wound care, and removing sutures or staples
Ability to manage patient communications via phone, online messages, and telehealth
Experience in supporting preventative care and chronic condition management
Ability to independently manage nurse calendars
Preferred
Experience in geriatrics and palliative care
Experience in a collaborative care setting
Participation in quality improvement projects
