Responsibilities
Provide nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor
Accept delegation from the RN/assigned supervisor in meeting the needs of the patient/family
Collect data through observation and communicate information to assist the RN and/or provider in patient assessment and care planning
Participate as a member of the health care team and accept delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family
Deliver care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care
Possess excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects
Qualification
Required
Licensed Practical Nurse (LPN) certification
Ability to provide nursing care under the supervision of a registered nurse (RN)
Experience in patient visits, including face-to-face and via telephone
Ability to review patient health history and medication
Experience in immunization administration
Ability to conduct focused nutrition assessments and blood pressure visits
Experience in telehealth interactions
Excellent communication skills
Ability to mentor and instruct
Participation in health care team activities
Knowledge of patient care plans and organizational policies and procedures
