Director of Case Management
TalentMatch LLC is a recruitment and staffing services company focused on providing customized solutions throughout North America. They are seeking an experienced Director of Case Management to lead a comprehensive, system-wide program in a community hospital, ensuring effective care coordination and utilization management during a period of organizational transition.
Responsibilities
Lead and oversee case management and care coordination operations
Assess current structures, workflows, and performance metrics
Identify gaps, inefficiencies, and risk areas; implement practical improvements
Partner closely with nursing, physicians, social work, and administrative leadership
Support patient flow, utilization review, and discharge planning processes
Guide teams through transition with clear communication and steady leadership
Ensure alignment with regulatory, accreditation, and compliance standards
Lead, develop, and supervise Case Managers, Social Workers, Case Management Coordinators / Discharge Planners, and Utilization Review Coordinators and Technicians
Design and operate an effective case management and utilization review program aligned with hospital mission and patient/family needs
Assess patient care needs and coordinate services to support safe, timely, and appropriate discharge planning
Integrate services across departments using continuous quality improvement (CQI) tools
Monitor and analyze utilization, LOS, denials, and throughput metrics to drive performance improvement
Ensure compliance with regulatory, accreditation, and payer requirements
Partner closely with nursing leadership, physicians, social services, and executive teams to remove barriers to patient flow
Use data, statistics, and trend analysis to identify problems and implement corrective action plans
Stabilize: Operate effectively in a high-pressure, post-acquisition environment
Address the elephant in the room: Address inefficiencies in patient flow, discharge delays & utilization practices
Lead: Lead teams through process changes while maintaining engagement and accountability
Support: Support system-wide stabilization efforts related to capacity, throughput, and financial performance
Qualification
Required
Licensed clinician in the state of practice (Maine)
Licensed Clinical Social Worker (LCSW): Grandfathered prior to April 1, 2015, OR minimum 5 years post-graduate experience from an accredited school of Social Work
Minimum 5 years of experience in a Case Management role
Demonstrated analytical ability for problem identification, data/statistical analysis, and performance evaluation through ongoing review processes
Basic to intermediate computer skills required
Certification in Case Management (CCM or equivalent)
Bachelor's degree (BS, BSN, or related field)
Current BCLS certification
Knowledge and experience with Milliman Criteria and InterQual Criteria
Preferred
LCSW with BCLS/ CCM certification preferred
Has already led case management teams in an acute care hospital
Understands how utilization, LOS, and discharge planning impact both patient care and financial performance
Is comfortable using data to drive decisions and accountability
Can lead teams through change without losing credibility or engagement
