Program Manager, Clinical Case Management
New England Center and Home for Veterans is seeking a Program Manager for their Clinical Case Management Program. The Program Manager will oversee daily operations, provide supervision to clinical case managers, and ensure Veterans receive appropriate services and support for their mental health and substance use needs.

Responsibilities
Oversee the Veteran 360 Clinical Case Management Program including the wellness and recovery group development process and the GPD Clinical Treatment and WORTH programs
Conduct weekly individual supervision with clinical case managers. Ensure clinical and administrative processes are adhered to, including completion of documentation and data collection by deadlines. Ensure services are provided within a Veteran centric and trauma informed frameworks
Work with the Transitional Housing Referral team to manage Transitional Housing referrals and ensure that Veterans are quickly matched with the Transitional Housing Program which best suits their needs. Oversee case load assignments
Work closely with the VA grant per diem GPD liaison and the VA WORTH liaison to refer Veterans to the GPD and WORTH Programs to ensure that programs are appropriately utilized to maximize programming resources. Intervene or provide guidance regarding clinical crises as needed
Work with the Director, Performance Monitoring and Evaluation and HMIS Manager to ensure data collection is accurate, timely and complies with agency and funding needs/processes. Prepare for and participate in VA inspections and site visits
Ensure GPD and WORTH requirements are adhered to. Prepare weekly report on Veterans currently served on the GPD grant and participate in weekly GPD meeting
Submit monthly GPD billing at the end of each month. Work with the WORTH Case Manager to ensure that the WORTH billing is submitted each month
Work with leadership regarding safety and security issues and crisis intervention. Inform and participate in the Review Board process
Work collaboratively with the Vice President, Human Services and other Human Services management staff to demonstrate proactive initiative and leadership to assist with program development and systems strengthening
Attend management meetings, and other meetings as assigned
Maintain a caseload of 3 to 5 resident Veterans. Ensure Veterans are swiftly and proactively connected with the services that will best meet their needs and goals
Provide individual supportive addictions counseling utilizing evidence-based interventions such as: Motivational Interviewing, Harm Reduction, Relapse Prevention and Trauma Informed Care
Manage crisis situations and intervene with caseload and with other Center residents when needed and alerted. Respond and follow up with event reports
Work in a collaborative manner with Veterans towards the goals of permanent housing placement, increasing income and benefits and improving self-determination
Complete biopsychosocial assessments, integrated service plans and discharge plans in conjunction with each Veteran, which best meets individual needs and program requirements
Facilitate addiction recovery and wellness groups for Veterans utilizing various treatment modalities and interventions
Work in a collaborative manner with Human Services and Operations staff to provide coordinated services
Ensure all data collection and documentation procedures are adhered to according deadlines
Provide community-based referrals to meet the needs of each Veteran during discharge planning to help maintain independent living. Provide appointment accompaniment as needed
Attend and actively participate in department, case conferencing, review board and program meetings as required
Supervise interns as needed
Perform other duties as assigned
Qualification
Required
Licensed Social Worker (LCSW or LICSW) or Mental Health Counselor (LMHC), or within six months of licensure
Two to three years' experience with Veteran population, chronic mental illness, substance use disorders, and homelessness
Strong assessment and crisis intervention skills
Case management experience
Strong leadership and analytical skills
Experience with Motivational Interviewing, Trauma Informed Care, Person Centered models of provision
Preferred
Knowledge of services in the greater Boston community
Knowledge of military culture
