Responsibilities

Provide support to clinical teams to ensure that patients receive chronic disease care and preventive care consistent with evidence-based guidelines
Perform and coordinate patient outreach to improve patient’s health
Directly support operational efforts for all population management initiatives that involve quality metrics
Assist practice teams in quality and process improvement initiatives
Utilize electronic registries such as Regional Health Information Systems, Arcadia Analytics, and eRecord dashboards for comprehensive patient care management

Required

AAS required
3 years in a professional office environment
2+ years clinical, operational, quality data collection and reporting experience
Basic MS outlook, Word and intermediate MS Excel are required
Excellent communication skills (verbal and written) required for interactions with patients and practice staff and telephone communication
Computer database navigation

Preferred

B.S. or B.A. degree preferred; or equivalent experience
Experience with LEAN processes and/or process improvement methods highly desirable
