Home Health Admin On-Call Coordinator
Cook Children's Health Care System is a leading healthcare provider focused on children's needs, offering advanced technologies and services. They are seeking a Home Health Admin On-Call Coordinator to maintain communication with clients and coordinate patient care.

Responsibilities
Maintains open communication as admin on-call coordinator with all clients, physicians, staff and referral sources
Follow up on all unresolved issues from office hours (i.e.lab results)
Coordinates new patient starts as required
Documents communications, referrals, and problems as appropriate and in a timely manner
Provide office staff with all pertinent information at onset of office hours
Qualification
Required
Graduate of an accredited school of Nursing
1+ year pediatric experience
Current Texas driver's license in good standing
Current CPR certification
Preferred
2+ years' experience in Home Health, Hospital or Critical Care nursing
Current National Certification (CPN or CRNI)
