Crisis Navigator

The Crisis Navigator role involves providing clinical and support services to consumers in crisis, assisting them in developing essential skills, and linking them to ongoing treatment and resources.

Responsibilities

Provide prevention/early intervention, care navigation, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position
Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider and family members as needed
Provide brief interventions using evidence-based techniques such as Motivational Interviewing, de-escalation, or other treatments appropriate to support consumer engagement
Document initial intervention, track patient follow-up, and clinical outcomes using an Electronic Health Record system (EHR). Document in-person and telephone encounters in the EHR, and use the system to identify and re-engage patients
Attend collaborative treatment team meetings and reviews of consumers
Provide therapeutic crisis intervention and emergency services as needed
Assist eligible consumers in gaining access to needed medical, social, educational, and other services
Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/followups for consumers
Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling, as well as technical expertise, to accomplish goals
Attend and participate in group sessions, in-service education and training opportunities, meetings, conferences,and special events in order to utilize opportunities for professional growth
Provide transportation for consumers in agency vehicles when clinically indicated
Work closely with outside organizations on resources and care coordination
Uses identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment

Required

Requires bachelor's degree in psychology, Social Work, or directly related field
6 months of experience working with adults who have mental illness, particularly those individuals diagnosed with a serious and persistent mental illness preferred but not required
Communication, organization, time management and clinical skills
Ability to work independently in a community-based setting
Valid Colorado Driver's license with a clean driving record

Bilingual Spanish preferred
