Trinity Health is a comprehensive provider of home care and end-of-life care, and they are seeking a Full-time Home Care Account Executive in the Montgomery County area. The Account Executive will act as a liaison between Mercy Home Health and various referral sources, coordinating care and processing referrals to increase market share.

Responsibilities

Act as a liaison between Mercy Home Health/St. Mary Home Care and referral sources such as discharge planners in acute care, skilled nursing, short-term rehabilitation settings, as well as independent living, personal care and assisted living facilities, CCRCs and physician practices

Work across all departments as necessary to coordinate care

Work closely with the referral source, patients and their families to outline services and gather necessary information for a safe transition to home

Process referrals and increase market share from assigned accounts to achieve target forecast
