Responsibilities
Patient Intake and Triage: Coordinate the intake and triage process for patients across multiple medical specialties/service lines, ensuring accurate collection of medical history and relevant information
Care Coordination: Navigate patients through the healthcare system and facilitate connections to appropriate specialists based on their needs
Multidisciplinary Follow-up: Manage complex follow-up care logistics and coordinate appointments for new/established patients, including labs, scans, and treatments
Exceptional Patient Service: Provide high-touch patient service, maintaining exceptional communication skills and fostering a positive experience for patients and families
Data Management: Utilize electronic medical record (EMR) systems and customer relationship management (CRM) systems effectively to maintain patient records, manage care transitions, and gather necessary medical documents
Communication: Act as a liaison between patients and healthcare providers, communicating appointment details and treatment plans effectively
Work with department leadership to coordinate patient appointments across multiple service lines or with complex care plans
Service Recovery : Address patient concerns and restore trust by providing exceptional service recovery and advocacy when issues arise
Qualification
Required
Associate degree required
instead of a degree, an additional two (2) years of experience in medical intake, scheduling, referral facilitation, or related duties
Minimum of four (4) years related experience (intake, scheduling, referral facilitation, care coordination) across two (2) or more service lines
Needs to have very good hospitality skills, be able to multitask, and be able to problem-solve
Logistics management capabilities
Patient advocacy and service recovery expertise
Teamwork and collaboration skills
Excellent computer skills; proficiency in Microsoft Office Suite
Preferred
Bachelor's degree preferred
