Make a Difference in Your Community with SSM Health - St. Mary's Hospital

Join our Transitional Care Clinic (TCC) and Infectious Disease Department

Provide continuity care with a focus on health and wellness for all patients, regardless of insurance or demographics

Conduct one-time, post-discharge visits

Work in an outpatient clinic setting

Collaborate within a multidisciplinary healthcare team

Qualification

PA/NP licensureInternal medicine experienceEmergency medicine experienceInterpersonal skillsCommunication skillsIndependent workCollaborative work

Required

Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program

Ability to obtain PA/NP licensure in Missouri

Excellent interpersonal and communication skills

Ability to work independently and collaboratively within a multidisciplinary healthcare team

Preferred

Internal medicine and emergency medicine experience is highly preferred
