Home Care Account Executive in Montgomery County
Trinity Health At Home is a national home care, palliative care and hospice organization serving communities in twelve states. The Home Care Account Executive will act as a liaison between the organization and referral sources, coordinating care and processing referrals to increase market share.

Responsibilities
Acts as a liaison between Mercy Home Health/St. Mary Home Care and referral sources such as discharge planners in acute care, skilled nursing, short-term rehabilitation settings, as well as independent living, personal care and assisted living facilities, CCRCs and physician practices
Is the face of Mercy Home Health/St Mary Home Care in their assigned geography
Works across all departments as necessary to coordinate care
Works closely with the referral source, patients and their families to outline our services and gather the necessary information for a safe transition to home; therefore, strong communication and relationship development skills are a must
Is responsible for processing referrals and for increasing market share from assigned accounts to achieve target forecast
Qualification
Required
BS/BA Degree in related field or business
Exceptional communication skills, both verbal and written
Ability to develop good rapport with referral sources and other community groups
Previous sales/marketing experience required
Experience in healthcare-related field with a proven track record
Computer efficient with working knowledge of Microsoft Office Suite
Current PA driver's license required
Preferred
Bachelor's Degree with 3-5 years healthcare sales experience
home care experience and/or clinical background preferred (i.e. Nursing, Medical Social Work)
